Preparing for Your Appointment

Once you schedule an appointment at one of our diagnostic imaging centers, there are a few things you can do to prepare for your visit.

Make sure you bring your insurance information, one form of identification, referral forms from your doctor or attorney, and completed patient forms. If you’d like to complete your patient forms before your appointment, you can click the link below to fill them out ahead of time:

We also recommend you bring contact information for your doctor or other healthcare providers. Once we have your test results, we will share them with your doctor immediately.

We recommend leaving any jewelry or other metal objects at home – especially if you’re getting an MRI. If you have any metal objects in your body from a previous procedure or surgery, please make sure to tell our staff ahead of time.

We recommend arriving at least 15 minutes before your scheduled appointment. So you can fill out any patient forms, and we can verify your insurance information or LOP coverage (if necessary).

When you arrive at one of our diagnostic imaging centers, you’ll check in at reception and fill out any forms, if necessary. Once the technician is ready, our staff will escort you to the exam room.

Then, you’ll change into a hospital gown and remove any metal objects you may have brought. Our technicians will explain the process and answer your questions before they start the imaging procedure.
